  8.     * Make Sure your FORM/Chunk names follow the naming conventions in
  9.       the document EA IFF 85 Standard for Interchange Format Files
  10.       (available from CATS) and also in the 1.3 RKM: Includes &
  11.       Autodocs IFF Appendix.
  12.  
  13.     * Call CATS at (215) 431-9300 to find out if you FORM/Chunk name
  14.       conflicts with any existing name.
  15.  
  16.     * Fill out this form.
  17.  
  18.     * Mail this form and and an AmigaDOS disk containing the ASCII
  19.       text specification of your FORMs/Chunks (plus any include files
  20.       and sample source code) to:
